Request For Online Section Enrollment
To register in the online sections, you must complete the form below and provide a documented evidence that you can’t attend the face-to-face section of these courses for good reasons, and it must be approved by the Department Chair. If you are registered in the face-to-face section, and you want to transfer to the online section, you must submit this form and get approval from the Department Chair.
